1. Table of contents not displaying for Visual Studio 2008 Help 2 integration

2. ASP.Net version doesn't work with IIS (ver 5)

3. HTML version of the help file only includes one namesapace.

All of this means that the produced help documents are unusable.

Help with these problems appreciated.

See the Help 1 and Help 2 Category property topics in the SHFB help file.  You may need to set the set the HelpAttributes correctly.  Also review the Deploying an HTML Help 2 File topic as it may help as well.  If you haven't tried it, use H2Reg as it works well in getting Help 2 collections registered.

As far as I know it should.  You may need to configure ASP.NET correctly and make sure it is registered.  Search Google for more info on registering ASP.NET for use with IIS.  If you're getting an error, you'll need to be more specific about what it is.

Are the classes you want documented public?  If not, they won't show up by default unless you enable the DocumentInternals and DocumentPrivates properties.
