Is it possible to customize the Table of Contents for the help file?

Maybe build the help file and save the intermediate files, customize the xml TOC, and compile it with the help compiler?

If so, what would be the steps to achieve this.

I ask, because a lot of times, I would prefer a different order to the table of contents for specific items in a namespace than just alphabetical. It would make more sense to me to have the 'Top-Level' class above classes it uses.

You can set the CleanIntermediates property to False and do a build.  All of the working files will be left in the .\Working folder under the OutputPath folder or WorkingPath folder if you set that property.  The default location if neither are set is .\Help\Working under the project folder.  You can then modify the Help 1 project files to your liking and run the Build1xHelpFile.proj or Build2xHelpFile.proj project using MSBuild to rebuild the resulting help file.

Another alternative is to write a plug-in that alters the table of content file during the build.  See the plug-in topics in the help file for more information on creating one.
